Privacy Notice

This notice describes how AmpuHealth Holdings, Inc. ("AmpuHealth," "we," "us") collects and handles information submitted through the AmpuHealth pre-launch waitlist form. It applies only to the waitlist. A separate, full privacy policy will govern AmpuHealth membership operations at launch and will replace this notice at that time.

Who We Are

AmpuHealth Holdings, Inc. is a Tennessee corporation. For privacy questions or requests, contact privacy@ampuhealth.com.

What We Collect

When you submit the waitlist form, we collect:

1. Your name
2. Your email address
3. Your ZIP code
4. The state derived from your ZIP code
5. Your phone number, if you choose to provide it (optional)
6. Your separate consent to receive text messages, if you opt in (optional)
7. The membership tier you indicated interest in
8. The exact consent language displayed at the time of submission
9. The date and time of your submission
10. The IP address from which the submission was made

We do not collect Social Security numbers, payment information, health information, or any other category of sensitive personal data through this form.

Why We Collect It

We use this information for the following purposes only:

1. To send you updates about AmpuHealth's pre-launch progress and launch readiness
2. To plan our geographic rollout and clinic network based on aggregate ZIP and state distribution
3. To validate our membership tier modeling against actual interest levels
4. To maintain a record of your consent

Phone Numbers and Text Messages

Providing a phone number is optional. We will not contact you by phone or text message unless you take a separate, affirmative action to opt in.

If you check the SMS opt-in box on the waitlist form, you are providing express consent to receive text messages from AmpuHealth at the phone number provided. Messages may include pre-launch updates. Message frequency varies. Message and data rates may apply. You may reply STOP to opt out at any time, or HELP for help. Consent to receive text messages is not a condition of joining the waitlist.

Phone numbers provided without SMS consent will be used only for occasional direct outreach by an AmpuHealth founder, if at all.

What We Do Not Do

We do not sell your information. We do not share your information with third parties for their own marketing. We do not collect payment from waitlist members. Joining the waitlist does not create a membership, an enrollment, or a binding commitment between you and AmpuHealth.

Service Providers

We use the following service providers to operate the waitlist:

1. Netlify, Inc. hosts the AmpuHealth website and processes form-related infrastructure.
2. Supabase, Inc. stores waitlist submissions in a secure database.

These providers process your information only on our behalf and only for the purposes described in this notice.

How Long We Keep It

We retain waitlist information until one of the following occurs:

1. AmpuHealth launches and you elect to convert your waitlist record into a membership
2. You request that we delete your information
3. AmpuHealth determines, in its discretion, that the waitlist is no longer active

If AmpuHealth does not launch, we will delete waitlist information within a reasonable period after that determination is made.

Your Choices

You may at any time:

1. Unsubscribe from waitlist emails using the link provided in any email we send
2. Opt out of text messages by replying STOP to any AmpuHealth text message
3. Request a copy of the information we hold about you
4. Request that we delete the information we hold about you
5. Request that we correct any inaccurate information

Children

The AmpuHealth waitlist is not directed to anyone under 18 years of age. We do not knowingly collect information from minors through this form. If you believe a minor has submitted information, contact privacy@ampuhealth.com and we will delete the record.

Changes to This Notice

We may update this notice as the waitlist evolves or as our service providers change. The version posted at ampuhealth.com governs. Material changes will be communicated to waitlist members by email.
