Choose your membership tier.

Select the tier that fits the components and access you need. Three tiers: Essential Access, Everyday Performance, and Lifestyle and Specialty.

Contribute monthly into the system.

One predictable monthly contribution funds your access. Contributions flow into the membership pool that supports component fulfillment for every member.

Submit a component request when needed.

When you need a component, you submit a request through your AmpuHealth member portal. Requests are reviewed against your tier's defined access.

Device is ordered through the provider network.

When a request enters fulfillment, the component is ordered through our prosthetic provider network. No paperwork loops. No reimbursement waits.

You get fitted.

Your in-network provider handles the fitting. The component arrives where it needs to be, when it needs to be there.

You move forward.

Your membership continues. As your needs evolve over time, your access evolves with you, supported by the same predictable monthly contribution.

Catalog items are pre-defined. No per-item prior authorization.
Membership runs continuously. No annual re-qualification.
